Print out this easy-to-use form, fill it in, and fax it to us prior to your visit. Be patient---it takes a few seconds to download!! Please write your appointment date and time on the form prior to faxing. Our fax number is 602-789-7702.

You can also bring your completed form with you to your visit---please come 10 minutes prior to your appointment time so we can input your information!!!
